Aged Care Assessment Team (ACAT) - Q.E.II Hospital
The Aged Care Assessment Team (ACAT) is a multi-disciplinary team whose primary role is the assessment of frail aged people over 70 yrs and some people with disabilities in their home, in hospital or in aged care facilities.
ACAT is responsible for assessing eligibility and approving applications for residential care (permanent and respite) Community Aged Care Packages and EACH Package. The team provides information and advice to elderly people in order to assist them in making informed decisions about care services. Self referrals and referrals from any appropriate source are accepted.
