This service is about supporting you to make your NDIS plan active.

We work with you to get you linked into the services and supports in your plan.

We are a registered NDIS Provider for 'Improved Life Choices' - which is made up of Support Coordination or Support Connection, or both. We've been delivering this service in the Barwon, Hunter and ACT trial sites for the past couple of years, so we have a great understanding of how to make your plan active and provide this service.
