About Us

We provide an extensive range of Community Health Services for all ages from children to older people, including: childhood immunisations, diabetes services, Asthma education, Hospital in the Home (CPAACS/HITH), palliative care, podiatry services, audiomerty services, Aged Care Assessment (ACAT/ACTIP/CCAP), cardiac rehabilitation and more. Our Community Nurses can also assist in general health services and provide referrals to other community health services.
