SchemeWise Pty Ltd is an NDIS registered provider.  We are experts for support coordination (to specialist level) in the ACT, Southern NSW (Queanbeyan and surrounds), and Murrumbidgee areas (Albury, Wagga Wagga, surrounds).  SchemeWise offers experience in complex disability, community and mainstream health environments.  Our team of health/allied health professionals provide you with trusted, quality assured and knowledgeable support to navigate the NDIS, make connections and put your NDIS plan into action. 

While we work with people from any disability background... our coordination expertise is particularly focused to neurological conditions like brain injury, spinal cord injuries, MS, PD, motor neuron disease, Huntington's, peripheral neuropathies, chronic pain, intellectual/learning/cognitive disability, deaf, blind, cerebal palsy, stroke, ALS, psychosocial problems and others.

SchemeWise provides community nursing, nursing clinical assessments to help with review and other processes (e.g. evidence) as well as counselling for participants, carers & families (general, trauma, mental health). We are also providing occupational therapy services and will be offering other allied health services from 2022 (see our website). 

SchemeWise is a people-first organisation, deeply experienced in understanding functional and capacity issues around various disability conditions, with the know-how to support linking customers to services/supports and living their best possible life.  Our people are also deeply experienced in child, education and juvenile justice matters as well as across multi-system interactions working to complex cases.  We work regularly with NDIS plan nominees, Guardians (public and private), public trustees, and others whom support people living with disability too.
