Our organisation, Australian Home Care (AHCS), is a disability care provider with 20 years of experience. Our person-centered approach ensures we provide a responsive and tailored service to all individuals requiring, in-home care, community access, rehab and respite services post hospital admission. We have extensive knowledge and history working with participants with disabilities such as Spinal Cord Injury, Acquired Brain Injury, Multiple Sclerosis, Cerebral Palsy, Neurologic Disorders, Intellectual Disability, Dual Physical and Psychosocial disabilities, and complex Psychosocial. We take pride in ensuring all our staff meet stringent qualification and experience levels in disability care services.

Our AHCS values enables us to specialise in providing effective services for more complex participants, such as supporting people from hospital to home, or alternative accommodation. We have extensive experience providing hospital discharge transition services and supports. We additionally offer bespoke packages for high complex care needs, SIL packages, and SDA residences.
