About Us

The 48 hour Follow Up program is a comprehensive program that provides support to Aboriginal patients who are being discharged from hospital. This service consists of a phone call or a home visit to the patient within 48 hours of being discharged to check on their social and emotional wellbeing. During this phone call or home visit, any issues or barriers the patient may be encountering are identified by the staff member conducting the follow up. Once identified, services and or referrals are put in place to help the patient. The aim of the program is to stop the unnecessary readmission of Aboriginal people into hospital.
