About Us

The Chronic Disease Program was set up to:

  • Ensure the coordination of regional and local level activities in chronic disease prevention so that resources are shared appropriately.
  • Develop key indicators and measures to support the implementation of QLDSCD in the non-government sector utilising ethnicity-related data.
  •  Identify gaps and inefficiencies in service delivery so that chronic disease funding can be targeted through evidence-based strategies and service delivery models.

In order to achieve the above objectives, ECCQ's Chronic Disease Program has to develop an integrated cross-sector approach to chronic disease prevention and management at the state-wide level which then links and supports work at the regional and local levels between different partners
