Southern Cross Care Client Engagement Team offers Consumer Directed Care (CDC) Packages Level 1-2 and Level 3-4
The packages aim to support you to exercise greater control over the care you receive, the type of service, who will deliver the service and when. It will be offered within a restorative or re-ablement approach to enable you to be as independent as possible.
The Client Engagement Team will manage/administer the budget under your direction. The Team offers Certified Case Managers, and offers uniqueness due to having developed core elements of person centeredness, offering:
- Respect and inclusivity, effective communication
- Trust and common purpose as well as shared vision
- Facilitation of the personal development of clients
- Client assessments, working effectively with clients, carers and families
- Advocacy, planning, monitoring outcomes and evaluation, all of which is purposeful
