Pre-planning
We can help bring your plan to life! Our Plan Management team offer a Pre-planning service that identifies the services of your choice and we will assist you to find service providers in order to access supports that best fit your situation. You will have access to a dedicated Community Linker that works with you to source pathways and programs that will enable access and inclusion within the community. Our Community Linker will work closely with participants, families and carers in an
effort to fulfill identified goals, hopes and dreams. Our Community Linker has strong local knowledge and works alongside community organisations supporting them to be welcoming and inclusive. Plan Management (Financial) We have an experienced, qualified plan management team led by our CPA registered Commercial Manager and financial team. It is our mission to offer individualised financial management in a meaningful
and transparent way.
Financial management activities include such things as:
• Analysing the funding streams and preparing
a budget that will enable you to fully utilise the funding available
• Working with providers
– to ensure their charges are aligned with your budget
– processing provider invoices
– paying providers
• Claiming payments from the NDIS
• Provide you with monthly statements that reflect
– How much has been received from your funding
– How the funding has been spent
• Together with the Participant, review the budget on an annual basis
Support Co-ordination
Our Support Co-ordinators are experienced and qualified. They will assist you to strengthen your abilities to coordinate and implement supports in your plans, and provide opportunities to access and participate in the community. The Support Co-ordinators monitor the quality of service provision and are there to listen to your needs and facilitate any desired or necessary changes to your scheduled supports. Our Co-ordinators are available 24/7 and they ensure a rapid response to meet your needs.
