Intensive Intervention Support
REAL Support Facilitators work with families in their homes. Support is tailored to assist Reunifications, Intervention with Parental Agreements, Kin and Foster Placements. Client-Centered Plans seek to address areas of concern with a view to implementing strategies, building skills to increase capacity and stability, repairing relationships, and sustaining the family unit. REAL Support can link families with a variety of affiliated services to ensure proactive and responsive service delivery.
Residential Placement Support and Independent Living Skills
REAL Support Facilitators assist Young People who struggling within residential placements, requiring assistance to transition to independent living models or stabilise in care homes. Facilitators work alongside existing supports, creating a collaborative group of service professionals to wrap around the Young Person holistically.
Youth Engagement: Outreach, Mentoring and Diversionary Support for Young People
REAL Support Facilitators assist Young People who are disengaged from their placements and schooling, at risk of homelessness, and under Youth Justice orders. REAL Support Facilitators ‘go to’ the Young Person; meeting with them to ensure general welfare and wellbeing is maintained. REAL Support Facilitators can link these clients with any necessary networks such as Centrelink, doctors, counseling, work experience items, and can transport to any necessary appointments (Youth Justice, job seeking, medical etc.). REAL Support Facilitators engage in diversionary and activity based-therapy (including access to trauma informed fitness program), role-modeling and mentoring, using a strengths-based approach to encourage Young People to succeed in their endeavours and redirect to more positive life choices and outcomes.
