About Us

RFQ Housing Support is an integrated wrap around support service for people living in boarding houses, crisis accommodation or hostels referred from the Mental Health Service. The program works collaboratively with the individual and their clinical services with a focus on an Individual Recovery Plan and supports that are inclusive of the following activities:

  • provision of up to three (3) months intensive support followed by up to nine (9) months of lesser intensity and frequency support where required
  • discharge planning and support to transition from bed based clinical services to boarding house, crisis accommodation and/or hostel
  • ongoing psychosocial support in relation to emotional needs and improving community and daily living skills such as self-care, cleaning, shopping, financial management, cooking and catching public transport
  • Support to access peer based group program
  • transition to secure and maintain stable tenancy and housing
  • facilitating access to specialist mental health, addiction, primary health services and social and community services
  • building and maintaining family and culturally appropriate support networks/community connections and activities
  • access to educational and vocational opportunities
  • access to the NDIS

Eligibility Criteria

  • Adults (18 years and over) experiencing a severe mental illness
  • Residing in a boarding house, crisis accommodation or hostel *
  • Accessing or recently (within the last three months) accessed mental health clinical care through the Mental Health Service
  • Referred by the Mental Health Service
  • Consent to participate in the program, including sharing of information between the services
  • Collaborative assessment of risk and safety planning supports the provision of supports in the community.
  • Not receiving support through the NDIS **

* Individuals who are “rough sleepers” are out of scope for this program

**an NDIS access request may be made over the period of support when supported by the individual, RFQ and clinical treating team
