Our programs provide individualised lifestyle support to people living with a disability in a diverse range of housing options such as a house, unit, group home or other form of dwelling.Support is tailored to an individual's identified needs and goals with a focus on developing their skills and their links within their community, and maximising independence and choice.The goal of this service is to enable people living with a disability to reside in community based settings.A group of carers or staff provide care and support to all individuals residing in an accommodation option and are directed by the individual plans that are developed with each person. Supports are provided to assist each individual to achieve their personal goals and may include:

•  Transportation to community based activities and appointments
•  Daily living assistance
•  Health and medical support
•  Community access and participation support
•  Skill development.
