Chronic Disease Management

The Chronic Care/Disease Management team works with patients in relation to general and individual health concerns, they also support patients to plan for health care and treatments associated with a wide range of chronic illnesses (e.g. Diabetes, Cardiovascular, Respiratory, and other chronic health problems). 

Client care is managed with efficacy by ensuring clients access clinical care, programs and services that meet the client’s care needs.  This role arranges case conferencing for clients and works closely with a range of service providers (internal and external) to ensure the best quality of care and services are provided to patients.
