Aged Care

The Aged Care Service is a multi-disciplinary team whose primary role is the assessment of frail aged people for services in their home. or for entering aged care residental settings. The Aged Care Service is responsible for assessing eligibility and approving applications for residential care (prermanent and respite) Community Aged Care Packages and Each Package. The team provides information and advice to elderly people. Self referrals and referrals from any other appropriate source are accepted.
