Aspire Community Solutions

Aspire Community Solutions are a Sunshine Coast locally owned and operated Support Coordination service. As carers for people with disability, we understand that everyone's journey is as unique as the support you require. We take a holistic approach, welcoming carers and other informal supports into the conversation.

We  have experience  in working with people of all ages, backgrounds and disabilities including: Psycho-social, Autism, Intellectual Disability, Acquired Brain Injury and those with High Medical Care Needs.  

We look at the whole picture, and tailor support solutions to fit your individual goals, beliefs and needs.  We will look for out of the box solutions. We offer the following services:

• Support Connection: We will support and empower you to be able to connect with informal, community and funded supports enabling you to get the most out of your NDIS plan and achieve your goals. 

• Support Coordination: We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community. 

• Case Management: Comprehensive management of all your supports, and services.  Case Management involves: assessment, planning, facilitation, care coordination, evaluation and advocacy.
