Specialist Support Coordination Queensland

Gray’s Support Services have capacity to accept new referrals for NDIS participants with specialist support coordination with high levels of complexity wishing to have support to access the community setting and achieve their NDIS plan goals using a holistic approach. Gray’s Support Services have backgrounds in Nursing, NDIS (internal interface), Mental Health both clinical and non-clinical as well as community health in indigenous communities.

Gray’s Support Services has experience working across multi level interfaces, including but not limited to Justice, Health, Aged Care, Office of the Public Guardian.

I have a high level of experience and creativity when sourcing potential service providers and resolving crisis that may arise. I am able to safely navigate service provision breakdown to ensure that each individuals support needs are met.

Service areas include, Ipswich, Toowoomba, Dalby, Warwick, Roma, Goondiwindi, Stanthorpe and all areas in between.
