About Us

Amity Health’s Integrated Primary Mental Health Care program is a fully funded service offering solutions focussed brief intervention for people living with mild to moderate mental health issues. The service is delivered across the Wheatbelt and the Great Southern and is a free service to those eligible.

Eligibility for Service:

1. Financial Hardship – Health Care Card not needed

2. Experiencing a mild to moderate mental health issues

3. Has a severe and persistent mental health issue, but not currently in crisis or in need of urgent assistance

4. Lives in a remote area where alternative services are unavailable (such as Better Access).

Amity Health will accept any GP referrals in writing but can access our referral forms on our website. All referrals are to be faxed to 9842 2798

All referrals will be triaged for appropriateness at Amity Health and then forwarded to location specific clinicians. We will have small clinical teams in towns throughout both regions with our goal to have a more widespread service model that is not centred in only one major regional town.


Locations of services will be at:

  • Wheatbelt:  Northam, Merredin, Moora and Narrogin
  • Great Southern:  Albany and Katanning.

The Stepped Care Model is not a one size fits all approach so clients will be able to move through the system both ways having access to services delivered in a number of different ways. 

Service options include:

Brief focused intervention - Face to Face or telehealth if client is in a remote area and Community support

Care management - For those with more chronic and complex problems who require specialised support in a case management style service. The Integrated Care managers will work very closely with GPs and WACHS in managing these clients.

Virtual care and telehealth - The virtual clinic aspect (PORTS: Practitioner Online Referral Treatment Service) is an evidenced based management tool designed by Macquarie University that allows for those with milder mental health issues to be managed quickly and efficiently. This will be a 2 way relationship between PORTS and Amity Health that will allow clients to be referred back to the Mental Health Portal if required.
