APM Local Area Coordinator - Success

APM  has been selected to deliver NDIS Local Area Coordination services in your community.
Our Local Area Coordinators (LACs) help NDIS participants, people with disability, families and carers to identify and access the support they need to enjoy a better life.
Our community-based teams also work with local businesses, organisations and service providers to better link and support people with disability.                                                                                                                                                                                                                                                                                                                            
What is a Local Area Coordinator?
APM’s Local Area Coordinators, or LACs, work directly with people with disability to help identify and assist access to the support services they need, including whether they could be eligible for the NDIS.
A Local Area Coordinator will help NDIS participants access the scheme and understand their plans.
For people with disability who are not eligible for the NDIS, Local Area Coordinators  can help them access other community and mainstream support services.

What we do

APM Communities helps NDIS participants and people with disability to identify and access the support they need. Our Local Area Coordinators work directly with people
with disability, carers and supporting family members. We help participants with access and people with disability who are not eligible for the NDIS. As part of our
local services APM Communities also provides practical advice, information and support to businesses, organisations and groups to improve services for people with
disability. We aim to increase inclusivity in the communities we work in through projects and partnerships that develop more opportunities for people with disability.
