Mosaic Community Care Inc. has been providing services for people with an intellectual and/or physical disability for over 25 years. From our early days as an organisation developed by community members looking for better outcomes for their family members, Mosaic has grown to employ over 200 people, all focused on providing contemporary, flexible and innovative services.

With over 200 staff, we support men and women with disability who have diverse needs and interests, and cultural and religious backgrounds. We have a long history of providing shared supported accommodation options and in recent years have broadened our range of services by establishing opportunities for people with disability to access supports to build their independence, alternative accommodation options, and community participation. Our vision is to “enrich the quality of life of people with disability” and we work in partnership with our Clients and the important people in their lives to explore opportunities, plan and shape services, and achieve personal goals.

What we do

Our Community Access and Support team provides tailored supports to assist people to live independently in their own homes or participate in activities of interest, this includes recreation, leisure, education and employment opportunities.

Our modern supported accommodation residences are located across the metropolitan area are staffed with dedicated, experienced teams providing 24/7 support in a welcoming, supportive, safe and homely atmosphere. We also support people to explore alternative accommodation options and build the skills to live independently.

Our qualified and experienced Positive Behaviour Support team support Clients who sometimes display challenging behaviour. The team seeks to improve the quality of life of people we support by working with individual’s to build understanding of why challenging behaviour occurs, and to developing strategies to meet unmet need.

Our team can assist people with disability to prepare for the NDIS and explore options, possibilities and potential.
