Uniting WA - Inner City

Street to Home - This is an assertive outreach program working with people who are rough sleeping mainly in the inner city area, and who are aged 18 or over.  Support Workers will build a rapport with people in the parks, streets, malls, etc with a view to linking them with the Inner City Service Centre where they can access a range of services.  Once the person is ready to make some changes, Support Workers can help them develop a plan using Outcome Star.  Support Workers will provide advocate, provide advice and information, make referrals, provide transport to appointments, and provide general support. Referrals: Referrals can be from any agency or individual. There is no formal referral form. Referrals can be by phone, email, etc.
