About Us

Strong Families is a planning and coordination process for consenting families who are receiving services from a number of agencies and where it is considered that the application of a more formal coordination process will assist in better achieving the desired outcomes for the family. At Strong Families meetings family members and agency representatives come together to share information and agree on actions to support the family in achieving their goals. The Strong Families Coordinator is the independent meeting facilitator and their role is to ensure the family and agencies are heard and a corresponding action plan is developed. A lead agency worker is also identified as the central point of contact for agencies and family members in relation to the action plan. Review meetings are then held at appropriate intervals.
