Avivo - Plan Management

Avivo is a private, not-for-profit agency, providing care for people who are frail aged, and people with permanent disabilities, to help them to continue to live independently in their own homes. Perth Home Care Service can provide most of its care on a subsidised basis through funding from the Home and Community Care Program, but also has a full fee private care program for those who do not meet the Home and Community Care criteria.
