City Of Swan - Community Care Services

This service provides help for eligible frail aged and people with disabilities living in their own or family's homes. Services offered include transport, home help, social support, in home respite and day centre, basic gardening and home maintenance.There is a fully accessible day centre for people with disabilities. Day Centres are situated in West Swan, Bullsbrook, Midland and Beechboro.

Activities: Adult Activity Club Day centre activities

Available To: Residents of Swan Shire who have disabilities, or are frail aged or have dementia, and their carers. All clients must be screen for eligibility for services
