Life Stream Foundation
Life Stream Foundation supports people with disabilities through individualised support and group programs to develop skills, independence, friendships and involvement in his/ her community through community based leisure and recreation in addition to independent living skills through in-home accommodation support which may involve sleepovers.
Community Access services can be provided through individualised support or small group programs:
- Disability Services funding packages
- Emergency & Crisis Care funding
- Commonwealth Respite and Carers Carelink funding
- Life Stream Foundation Recreation Access funding - funded by Disability Services Greater Brisbane
- User pays programs
In-home Accommodation Support can be provided on the basis of any of the above funding options.
Leisure programs are facilitated by volunteers on a 1:5 support ratio. People with higher support needs are welcome to attend programs however he/ she may be required to bring a carer. A program guide is produced every 4 months.
Leisure programs include:
- weekend leisure eg. movies, dinner, bike riding, football games
- special events eg. concerts, festivals
- theme parks
- social club
- weekends away
- regional and interstate holidays
- international holidays
