About Us

Aged Care Service is a multi-disciplinary team whose primary role is the assessment of frail aged people or people with disabilities, in their home, in hospital or in aged care facilities. Aged Care Service provides assessment of clients' physical, psychological and social needs and matches these needs with available services. Aged Care Service is responsible for assessing and approving applications for residential care (permanent and respite), Community Aged Care Packages and EACH Package. The team provides information and advice to elderly people and people with disabilities in order to assist them in making informed decisions about care services. Self referrals and referrals from any other appropriate source are accepted.
