This role will enable clients, families and carers to have a better understanding of service options available to achieve recovery goals and manage their condition. It will be responsible for identifying service gaps that exist and potential innovative solutions to barriers for service access for a range of GPs and service providers staff to have a better understanding of services available in their regions to support the social, mental, and physical health needs of clients with severe mental illness and promote effective multi-disciplinary care.
