Make sure you come and see the Pharmacist if:

  • You have recently been in hospital
  • Have had a change in medication
  • You want to get a Webster pack organised
  • You have any chronic disease
    • Diabetes
    • Heart disease including Heart failure, heart attack, high/low blood pressure
    • Kidney disease

CRAICCHS does not give out medication but can help organise it with the local pharmacy.
