About Us

Bridges Reconnect is a federally funded early intervention program that provides intensive case management support to young people between the ages of 12 and 18 years of age old who are at risk of homelessness, or have recently become homeless. Bridges Reconnect operates in an outreach model of service delivery, providing a free and confidential support service to young people and their families that meet their eligibility criteria. Bridges Reconnect  aim’s to support young people and their families (where appropriate) to: stay at home until they have the necessary skills to become independent, connect with emergency support services if they have become homeless, re-engage young people with their communities, education and families (where appropriate) through counselling, mediation and general support, and learn how to better deal with conflict within family home.
