Care Coordination Consultancy was developed due to a high demand in the market for quality services supporting those with a disability.  

Our professional team at Care Coordination Consultancy consists of Skye Hartung (CEO), BaPsychSc MMentHPrac, who has over fifteen years of experience working with people to recover and rehabilitate within the community, acute, and forensic settings. Our Psychosocial Recovery Coaches and Support Coordinators are all qualified with experience. Furthermore our Recovery Coaches have lived and learnt experiences of mental health creating connection with our people. We provide Support Coordination, Specialist Support Coordination, Psychosocial Recovery Coaching, Innovative Community Access and Supported Independent Living (SIL), working alongside people with complex behaviours within the community, acute, and forensic settings.
