Journey Coordinators operate as independent and consistent partners, via a range of local partners across the Tablelands, supporting a person's care journey and helping to navigate the mental health and broader service system. Service focuses on capacity building, support, psychoeducation, navigation and coordination of referrals as well as a range of other low intensity supports.
