About Us
Living Hope Disability Services is a registered NDIS provider located in Melton South, Victoria. We provide person-centred disability support services to NDIS participants across Melbourne's western suburbs and surrounding areas. Our services are designed to assist people living with disabilities to increase their independence, participate in their communities, and work towards their individual goals.
Living Hope Disability Services supports adults with physical, intellectual, psychosocial, and other disabilities who are eligible for NDIS-funded supports. Services are tailored to each participant's needs, preferences, and goals, with a focus on dignity, inclusion, and wellbeing.
Key activities and supports include:
• Assistance with daily living and personal care
• Household tasks and domestic assistance
• Community participation and social activities
• Transport assistance for appointments and community access
• Support coordination
• Supported Independent Living (SIL)
• Respite and short-term accommodation supports
• Skill development and capacity-building activities
• Individualised support in home and community settings
Participants must have appropriate NDIS funding or be able to privately fund services. Support eligibility and service availability are discussed during the intake process to ensure services meet participant needs and goals.
