Wimmera Hospice Care - Wimmera Health Care Group

Wimmera Hospice Care is a palliative care service that supports people living with life limiting illness and their families and carers. Our team supports patients at home, in aged care facilities and whilst in hospital. The Wimmera Hospice care team works with the patient's local doctors, nurses and allied health care team to provide care. Main Services: Wimmera Hospice Care nurses help with managing symptoms and co-ordinating care. Specialist palliative care doctors and nurse practitioner from Ballarat may visit patients in the Wimmera. A range of equipment is available often free of charge to patients and carers. Education sessions are provided to patients and their carers, health care professions and community groups. A bereavement support program is offered to family/carers.

To ensure that all people who require palliative care receive care that is appropriate, sustainable and responsive to their needs. To promote Community Awareness of Palliative Care

Cost: No charge for medical & nursing consulation. All other service providers charge fees according to funding structure ie, HACC, PAC, HITH etc

Referral: Referrals are accepted from patients, family members/carers and health professionals (providing consent is given by the patient/legal representative)
