Welcome to Centacare FNQ Support Coordination services.

Once you have your NDIS plan, a Support Coordinator will support you to utilise your plan to best meet your needs and guide you to use your plan to achieve your goals. Whether that be to live more independently, increase your skills or be included in your community and in employment, Centacare FNQ is here to help. Our support coordinators value the diversity of our community and work with everyone.

We support you to:

• Understand your plan and what supports and services you can pay for with your NDIS funding.
• Put in place the service agreements and supports to meet your needs and achieve your goals, with the providers of your choice.
• Connect you with community, formal and informal supports, and other government services, and
• Answer any questions, concerns or as a general point of contact if something in your life changes.
