Home and Community Care (HACC) - Longreach

HACC programs is a joint Australian Government, State initiative.The program provides funding for services which support people who are frail aged, younger people with disability and their carers, who live at home and whose capacity for independent living is at risk or who are at risk of premature or inappropriate admission to long term residential care. Home Care Services provide domestic assistance, social support,,respite care, yard maintenance, dementia and /or carer counselling/support, information, advocacy and transport ('door to door' in our wheelchair accessible bus or other vehicle, to appointments, shopping, social activities etc. Monday and Friday - Townsville, Tuesday, Wednesday and Thursday - Local Areas). Telephone (07) 4783 7015 to make bus bookings. Access to services is based on assessed need. You, your doctor, a family member or friend can contact the Service to discuss your needs. Clients are asked to pay a small contribution for most services. Special consideration is given to people with limited finances.
