About PIR
Sunshine Coast Medicare Local and our partners have been asked to establish the Partners in Recovery initiative. An investment of $8.8M over the first 3 years will be used to provide better co-ordinated responses for people with severe and persistent metal illness by joining up the range of supports and sectors to provide a wrap-around model of care.
The PIR model strengthens coordinated responses between partners and the wider community, improving collaboration between agencies as varied as housing and homelessness, employment, education and transport. This initiative provides fundamental reform in the way we address improving health outcomes of some of the most vulnerable people in our community. PIR builds on the work established though the Sunshine Coast Hospital and Health Services’ Care Coordination Model. Care coordination has been recognised as a best practice model and our communities will now benefit from significant Australian Government investment.
Sunshine Coast Medicare Local is the lead agency for the PIR consortium in the Sunshine Coast Medicare Local region. The 13 partners come from health, housing, mental health, Aboriginal and Torres Strait Islander health, drug and alcohol, aged care, disability and carer organisations. We are working together to identify strengths and common goals to provide seamless and integrated pathways.
