TAIHS has partnered with Deadly Choices and the Registry of Births Deaths and Marriages to provide kids aged up to 5 years with a FREE Deadly Choices Birth Certificate package worth $63!

The commemorative package includes a limited-edition Deadly Choices birth certificate design (choose from Brisbane Broncos, Gold Coast Titans or North Queensland Cowboys inspired designs) PLUS a legal birth certificate which bub will need for things like enrolling in school and applying for a driver’s license.

There are four steps to getting your birth certificate:

1. Check that bub is up to date with their health check and vaccinations
2. Fill in a birth certificate package application form available from the clinic
3. Return it to the clinic or to the address detailed on the form
4. Births, Deaths and Marriages will arrange to post the certificate package to you at the address you nominate.
