Diabetic Care Program

The TAIHS Diabetic Care Program aims to improve the health of clients with diabetes through education and care management strategies that improve a person’s diet, physical activity, and weight management.  The program also aims to enable a client’s self-management of their chronic illness.

Working closely with the Doctor and the Chronic Care Coordinator, clients can access the following diabetes services:

• Full health check, assessment, screening, and testing including blood sugar levels, HbA1c, urine ACR and cholesterol
• Foot checks and eye health checks
• Diabetes education and advice about nutrition
• Care planning and ongoing medical advice

Additional service and group

TAIHS also invites clients to join the Got Sugar Diabetic Group.  The group meets on a regular basis, they do fun activities around healthy cooking, and they share recipes and their own personal experiences about managing their healthcare.  For more information about the group please call Aunty Emily on 4759 4000.
