Support coordination

Our Model

The NDIS can be confusing and often leaves people feeling overwhelmed. Our support coordinators are here to help clients and their families understand and navigate the NDIS, not only providing impartial advice on which NDIS services and providers will best suit their needs, but actively creating and implementing tailored NDIS plans for clients.

It’s a tailored partnership and service that, throughout regular reviews, ensures every client is able to fulfill their potential and acheive their life goals.

Support Connection

Providing short-term assistance to implement your plan by identifying service providers and negotiating Service Agreements.

Support Coordination

Our primary role is to support you long-term to navigate the NDIS and all its complexities. In doing so, we will work in-depth with you to understand, prioritise and implement your NDIS plan. The ultimate goal of our support coordination is to maximise your plan by identifying your goals and aspiration.

Specialist Support Coordination

Providing specialist assistance when your needs and NDIS plan are more complex. Our Specialist Support Coordinators recognises the importance of the right relationship with you and therefore our team of Allied Health Professionals and Social Workers will work with you to resolve crises and complex situations and implement them in you plan.

 

Therapy services

Therapy services team

Our diverse team of experienced allied health clinicians, help clients achieve goals and reach their potential. We offer a range of therapeutic services, either individually or as a holistic co-ordinated program. With evidence-based assessments and ongoing therapeutic support, services are delivered at our local consulting rooms or in clients’ homes.

Occupational Therapy

Occupational Therapy supports individuals by building skills and confidence in taking part in a range of everyday activities, including:

  • Building skills for independence at home, day service, work or leisure activities
  • Enabling meaningful participation in the community and group activities
  • Accessing specialist equipment and giving guidance and support with home modifications
  • Adapting to life transitions such as decreased mobility or ageing.

Positive Behaviour Support (PBS)

PBS is focussed on increasing quality of life by determining the reasons behind challenging behaviours. Our PBS Practitioners can assist you with:

  • Functional Behaviour Assessment
  • Development and Implementation of Behaviour Support and Response plans
  • Direct intervention and skill development
  • Provision of training for support people
  • Monitoring and review of plans and interventions

Psychology

Psychologists work alongside families, educators and direct workers to support client’s learning, development and participation:

  • Behaviour support and assessment
  • Emotional and relationship support
  • Social skills
  • Transitions
  • Specialised individualised therapy
  • Counselling
  • Relationships (human relations counselling and education)

Music Therapy

Music Therapy uses music in different ways to promote and support health and well-being outcomes within a therapeutic relationship. Clients of all abilities and skills work towards a number of goals individually or in groups which may include:

  • Improving communication skills
  • Increasing opportunities for social engagement
  • Encouraging emotional expression
  • Building meaningful relationships through music.

Speech Pathology

Speech Pathology assists individuals to communicate to the best of their ability and to safely swallow food and drink.  Clients may specifically work on:

  • Learning and developing new communication skills, both verbal and non-verbal
  • Communication training for families and support people
  • Using alternative communication techniques
  • Accessing specialist equipment and services
  • Advice and assistance around mealtime support.

Therapy Assistant

Our Therapy Assistants work directly with clients under the supervision of an allied health professional to help achieve therapeutic goals. Their input may include:

  • Transferring and consolidating skills to increase independence across all settings
  • Increasing community participation and social engagement
  • Practicing and developing communication skills including alternative communication techniques
  • Establishing routines and strategies tailored to improve overall development
  • Using aids and equipment to increase independence.

 

Greensborough Community Campus

What we offer

Our campus

At our Greensborough Community Campus we support individuals to reach their full potential. Our vibrant, purpose-designed facility at Greensborough offers a range of innovative studio-based activities on site, such as art, music, living skills and ceramics.

Our Welcome Lounge and garden space offers a hub for social interaction amongst clients and the opportunity to develop friendships and enhance wellbeing. We offer diverse, flexible group programs supported by a team of professional staff. We are able to assist on a one- to-one basis in the designing and delivering of tailored support arrangements of your choice.

Wellbeing and Life skills development

We support people to develop life-long skills in activities of daily living delivered in community- based settings or provided in one of our purpose built studio facilities. Within our life skills area we offer garden-to-recipe experiences that include on–site gardening, food preparation and cooking.

Our creative arts, craft and pottery programs, all in purpose built studios, provide clients with the opportunity to develop skills and achieve recognition for their talents and creative works.

Within our wellbeing area we offer assistance and support in personal care and grooming, meditation, sensory programs, physical exercise activities and movement to music. These fulfilling, calming and engaging programs increase a person’s sense of value and wellbeing.

Education and Learning

Both certificate based education and developmental training is provided to enhance skills and knowledge. Our developmental courses offer a media program that aims to promote a person’s communication, numeracy skills.

Education related to travel-training supports independence and safety in the community. We also provide planning and support with financial and transitional life arrangement, personal care and daily life skills development.

 

Sydenham Community Campus

School Leaver Employment Supports - My Future

 

My Future

The journey from being a student at school to becoming an adult at work is different for everyone; a significant milestone but, with the right support, it can be an exciting and safe adventure.

My Future aims to support school leavers on their journey from school to work by progressively introducing them to a range of opportunities to try different types of work, develop personal skills and confidence, and learn how to succeed in employment.

Our Model

We start engaging with students and their families in their last year of school, we work on building a relationship and planning together how your future supports will look.

My Future is led by our Employment Coordinator and is supported by Community Support Workers, and Educators at Kangan Institute. Providing a pathway that is tailored to meet individual abilities, preferences and aspirations. Our new model provides you the supports, skills and the opportunity to be ready for a valued role in your community.

Together with our Employment Coordinator and Community Support workers, a safe and supportive environment is our focus to develop the right skills.            

Outcomes

Our point of difference is that we also offer an Accord Employment Service, operating a successful Disability Employment Service (DES) that can directly support My Future clients to find and keep regular paid work when they are ready for open employment.

SLES funding

The School Leaver Employment Support (SLES) funding is an annualised funding model. This model provides maximum flexibility in the delivery of supports:

• SLES offers you individualised support for up to two years

• SLES is NDIS support available to school leaver’s who have a goal to work in employment

• An exit report is completed by Accord at the end of the two years or on exit to a DES, whichever is earlier

• This is for the school leavers’ who have employment in their plan but not suited for DES pathway straight from school.

My Future provides you the opportunity to be ready for employment.

Disability Employment Services

What we do

Accord Employment helps job seekers with a disability along their journey to a rewarding career.

Our job seekers are as diverse as the general community and have all types of abilities. We assist qualified professionals, enthusiastic assistants and hard-working school leavers.

Employers and work experience hosts are key partners in achieving positive outcomes. We work in partnership with you the employer, and job seekers to ensure that we meet all your business needs.

How we work

Our careful job matching begins with getting a clear understanding of your recruitment expectations. Our staff are experienced in efficiently analysing your workplace and job requirements and then matching them to a job seeker’s skills and interests.

We are onsite to assist with on the job training from day one until your new worker has settled in and is working well. Then we provide ongoing support for as long as needed and we continue to stay in regular contact to ensure the work is always done properly and that any issues are resolved promptly.

Subsidies

• Wage subsidies are available to reward you and offset the cost of spending a little extra time with us and your new worker.

• Workplace modifications or specialised equipment are sometimes required, and we will arrange government funding to cover approved costs.

• The government’s Supported Wage System is included in all Modern Awards and allows workers with a disability who are unable to work at full productivity to be paid a reduced wage commensurate with their work rate.

Assistance for existing workers

Some people are born with a disability while others acquire or develop a disability in childhood or adult life. It can happen to anyone.

Our Work Assist program helps existing workers who are experiencing workplace issues that relate to a disability or health condition to maintain their employment. The program can provide extra support, training, modifications or equipment.

Cost

All our services and supports are government funded and free to job seekers and employers.

 

Short Term Accommodation

Our Short Term Accommodation (STA) enables people with a disability to experience living away from home in a supportive environment. STA provides time-limited breaks for parents and carers of people with a disability on an episodic basis, to support and maintain primary relationships. Our team supports individuals in a person-centred environment. We provide a professional and innovative service that promotes choice, personal development and life experiences, helping people to achieve their unique goals and aspirations throughout our approach of achieving together.

Accessing our Short Term Accommodation

Frankston

Our service is based in Frankston and provides short term accommodation for people aged 16-25 years who live at home, have behaviours of concern and live within the Bayside Peninsula area.

The house is purpose-built with five bedrooms and has the capacity to be split into two separate units with separate living areas. Activities include community outings, in-house games, sensory activities, art and craft and movie nights.

Bookings are scheduled in advance and cater for different lengths of time, usually 1-3 nights at a time.

In assisting families/ carers, transport may be provided from home to school or services at an additional cost and dependent on proximity to the service.

NDIS participants will need to ensure that they have adequate Short Term Accommodation (STA) at the 1:2 rate to cover service costs.

Macleod

Our service is based on a relationship model where individual harmony is paramount to the success of support delivered.

Our short term accommodation located in Macleod is available to people up to the age of 64 years, who live at home.

Bookings are scheduled predominately for 2-3 night stays but can be negotiated outside of this.

In assisting families/ carers, transport is provided from home to school or services at an additional cost.

The house is purpose-built and has the capacity to be split into two separate units. The house is quite spacious comprising of five bedrooms with a TV in each room. There are two living rooms, a main kitchen with dining room and a smaller kitchenette in the rear unit of the house.

 

Supported Independent Living

You will be at the centre of everything we do, with support arrangements tailored to your independent living needs.

Accommodation - Supported Independent Living

Supported Independent Living (SIL) allows you to live independently in shared supported accommodation. We offer flexible support services to live independently, achieve your goals and to participate as valued active members of the community.

We will support you with daily living skills, travel, individual and group outings.

While we recognise the importance of experiencing independence, we will support you to gain confidence to build lasting relationships and achieve your desired outcome.

Factors for consideration

• The applicant is aged 18 years or older

• The applicant is eligible for and is able to secure an appropriate level of NDIS funding/or equivalent funding

• The support model is suitable to meet the individual’s support requirements.
