About Us
The Community Support Program provides a personalised support program to people who have a diagnosed mental illness and wish to live independently in the community.
One full-time dedicated worker provides case management services, home visiting and supported referral services. In addition there is 24 hour emergency crisis telephone cover provided. The service has capacity to support up to 20 clients of differing complexity each year. The level and duration of support provided varies according to the unique needs of each person.
For people living with a mental illness, a stable and affordable home is a fundamental enabler for recovery. Adequate housing increases quality of life and reduces the risk of relapsing into illness. Housing support is delivered through a number of Department of Communities – Housing units managed by 55 Central. The Community Support Worker provides support around tenancy management, independent living, personal development and budgeting.
