About Us

Home Links is a multi-disciplinary service aimed at facilitating safe and early discharge from Bunbury hospital and avoidance of unnecessary hospital admissions. Home Links Services are part of a range of ambulatory care services offered by Bunbury Hospital to provide acute nursing services, allied health services, education and interim support. 

This allows patients hospital treatment to be continued at home or in the community. Home Links Services aim to reduce in-patient length of stay, reduce re-presentation to Emergency Departments, avoid hospital admissions, provide care closer to home and ensure patient centered care is provided.

Services are provided 7 days/week, in home or clinic appointments in the Greater Bunbury region (30km radius from Bunbury)

Services include:

1) Ambulatory Nursing Services (ANS) provides acute nursing care, education and support, enabling patients to continue their hospital treatment at home. Care may be delivered in person, by Telehealth, by phone or in the clinic.

Services include:

  • Orthopaedic Post Discharge Follow Up
  • Hip Fracture Follow Up Clinic Information
  • Inpatient Follow Up Clinic Information
  • Discharge Lounge Information
  • Wound care
  • Drain management
  • IV Antibiotic administration
  • Medication management

2) Rehabilitation In the Home (RITH) provides a short to medium term (1-6 weeks) of allied health therapy such as Physiotherapy, Occupational therapy, Speech Therapy, Dietetics, Social Work, Allied Health Assistant and Liaison Officer for patients in their homes post hospital discharge. The service aims to support early discharge from hospitals or to avoid unnecessary hospital admissions.

3) ED Links provide short term allied health services in Bunbury Emergency Department with the potential to follow up clients for rapid support in the community. This includes Physiotherapy, Occupational therapy, Speech Therapy, Dietetics, Social Work, Allied Health Assistant and Liaison Officer. The service aims to prevent hospital presentations and admissions.

4) Interim Hospital Packages (IHP) provided in collaboration with Bethanie, this is designed to provide low-level support services, such as personal care, shopping or domestic assistance in the patient’s home, to enable a safe and timely discharge home. The packages are short-term, temporary and aim to bridge the gap between hospital and other ongoing support.
