At Amity Health, we understand that living in country regions can make accessing allied health services and programs challenging. In the South-East Coastal Goldfields region, we provide the following service to clients who have, or are at risk of developing, diabetes, heart disease, or respiratory disease.

Integrated Chronic Disease Care Program:  We provide Care Coordination and allied health services to support clients to manage their health conditions. The program requires a GP referral and is for clients who are disadvantaged or living in an area not adequately serviced.
