About Us
Phoenix Place Inc. assists people in
the Mackay area with a mental illness, who wish to receive support for their
recovery. The Recovery Model encourages individuals to be actively
involved in their own recovery and provide the direction and support needed to
help them achieve their own goals.
Phoenix Place Inc assist clients to
achieve their desired levels of independence and improve their ability to
participate more fully in their community. Phoenix Place Inc. links individuals
with a disability into their defined "Communities of Interest", help
build strong interdependent relationships within these communities, then when
appropriate "fade out" to enable strong community based relationships
to further evolve.
Phoenix Place Inc has a number of funded
programs:
*Block
Funded Services apply directly to Lifestyle Support Program. Waiting lists
and priority ratings apply. Services include Community Access, Community
Linking and Lifestyle Support. Persons from 10 years of age can access this
funding.
*Adult
Lifestyle Support Program contributes funding and support to help meet the
disability support needs of individuals. This funding and support is intended
to complement informal networks and promotes access to other general community
services.
*Project 300
is a Queensland Government initiative that aims to improve the well being
of people with a psychiatric disability transitioning from long stay
institutionalised care to the their local communities. Project 300 brings
together housing, disability supports and clinical supports to improve the
quality of life of people eligible for Project 300 support.
*Housing and
Support Program supports people with a psychiatric disability to transition
from Queensland Health extended treatment mental health facilities or acute
care and integrate back into their communities. This is achieved through a
collaborative partnership between The Department of Communities - Disability
Services, Queensland Health and Housing and Homelessness.
