Team Care Australia (TCA) is owned and operated locally on Bribie Island and Caboolture, offering personal care to our community. We form part of a network of local businesses that work together to achieve optimal levels of care our community deserves.
Our team are actively engaged in building local links and developing connections, and are dedicated in treating each person as an individual. Our team are comprised of skilled and experienced professionals who are compassionate, understanding and are very interested in taking the time to get to know and learn about you.
 

Why Use Us!

At TCA we work with you, for you with your best interests at heart in every step of our relationship. We can tailor our services to your requests. Your Service Co-ordinator will work with yourself and your family to customise and build a programme suited to your needs and budget. We will work with you as a team to design and deliver support, suited to your preferences in order to maintain your independence.
We can offer services to complement your existing funded services on a needed basis, with no locked in contracts so you can start, pause or stop our service as you need them. Our services can be accessed without an ACAT assessment, enabling care to commence in emergency situations with no delays using a fee-for-service agreement.

Our team will work alongside you and your family in creating a programme of care that includes services that will assist you to achieve your personal goals in living life as independently as possible. At TCA, all care is delivered under the planning, direction and supervision of an experienced Registered Nurse. We offer a range of services in the comfort of your own home.

Our nursing services are provided by our team of Registered and Enrolled nurses. They collectively have experience in primary health, aged care and are committed in working with our clients for our clients to meet individual needs.
• Specialised assessment
• Health promotion
• Complex personal care
• Diabetes management
• Dementia care
• Continence management
• Wound consultation and management
• IDC/SPC management and TOV
• Post-acute care and transitional care
• Stoma therapy

 

Our team of Personal Carers all hold a minimum Certificate 3 in Aged care. Personal care services provide assistance with daily self-care tasks in order to help our client maintain a quality of life in the comfort of their own home.
Our carers provide assistance with daily living including:
• Hygiene assistance, dressing and grooming
• Assistance with self-administration of medications
• Assist with Mobility
• Assistance in Managing Incontinence
• Assist with Communication
• Pressure Injury prevention
• Compression stocking application
• Assistance in Transitioning from hospital to home
• Palliative Care
• Escort to appointments, shopping or social outings
• Respite


Domestic Assistance

• Cleaning – thorough or spring clean
• Laundry and ironing
• Change linen and make beds
 

Respite Care

Our services involve a personal carer who comes to your home, so your carer can go out for a few hours, or they may take a person in care for an outing for a few hours.
 

24 Hour On-call Services
Clients and carers may request 24 hour access to a Registered Nurse in times of need via our on-call service package.

 

Social Support

Our team offer clients an opportunity to participate in social support activities in order to promote health and well-being. We can provide one on one interactions and initiate activities suited to our client in aim to promote social wellness.

• Assistance in preparing grocery lists
• Assistance in general shopping needs – accompanied or unaccompanied.
• Plan and assist meal preparation
• Monitor diet, eating and food expiry dates
• Assist with pet care and care for plants
• Pick up prescriptions, medication reminders
• Companionship and conversation
• Run errands and pay bills
• Emotional support
• Social Calls

 

Accessing Our Services

Our services are available privately without an ACAT, on a fee-for service basis and are negotiated on individual circumstances. We can provide emergency care, short term care or used to compliment your existing services.
For eligible DVA clients, we accept direct referrals from your GP or other health professionals. Please make contact with your GP so we can be in touch with you.

How much do I pay?
Both TCA and Government policies ensure a fair and equitable approach to the charging of fees. The amount you pay depends on whether your care is regulated and subsidised by the government, by another funding source, or if you are paying privately for your services. Some clients may be expected to contribute to the total cost of their care.
