Linkages: Central Highlands Coordinated Community Care (Ballarat)

Linkages Centeral Highlands Coordinated Community Care provides case management and an arrangement of services to support an individual's choice to remanin living at home. These services are provided in either a Home Care Package (HCP) or a Linkages Package format and promote an individual 's dignity while contrinbuting to his or her physcial, emaotional and social wellbeing. Linkages provides Level ! to Level 4 HCP's

Cost: In most cases consumers will contribute a small amount to their care. Linkages will work with the consumer to determine the correct fee.
