The Aged Care Assessment Team provides assessments of frail older people with the aim of assisting them to maintain their independence, maximise in home support and access ongoing services.

Aged Care Assessment Team - ACAT

The Aged Care Assessment Team provides assessments of frail older people with the aim of assisting them to maintain their independence, maximise in home support and access ongoing services.

Services

Assessments and approvals for:

  • nursing home
  • hostel
  • transition care
  • community care and flexible care packages
  • assisting clients to gain access to the most appropriate care services to meet their needs.

The team services all hospitals in the Gold Coast area providing assessments and approvals for inpatients who are medically stable and who require Transition Care and permanent placement.

Younger people with disabilities are eligible to apply to be assessed by the Aged Care Assessment Team after all other appropriate care options aimed at younger people have been exhausted.

Referrals

Referrals can be made by phone, fax, post or email from any source including self-referral.
 

The Aged Care Assessment Team requires that the client, the carer, next of kin, or the Enduring Power of Attorney has consented to the referral.
