About Us

The Cancer Care Coordination service primarily focuses on the care of patients in the outpatient setting whilst on active treatment and in the acute follow-up phase or in transition to palliative care.

Patients are predominantly reviewed in formal outpatient clinics in the, Cancer and Blood Disorders Outpatient Clinic.

Patients are subsequently referred to the appropriate services to ensure continuity of care during survivorship. However, the Cancer Care Coordination service will accept referrals for inpatients and patients in the survivorship phase where supportive needs are identified.

Referrals to the Cancer Care Coordination service are submitted by all members of the multidisciplinary team and patients may also refer themselves to the service.
