About Us
This program aims to remove barriers to accessing and maintaining treatment by providing flexible and innovative local referrals to health professionals and support agencies that have been vetted and approved by QPP to provide safe, confidential and respectful services. If you are struggling with your housing, finances, mental health, or other issues, our Case Managers can help link you to services to ensure your HIV management stays on track.
This program recognises that a client’s health can be influenced by a wide range of social determinants such as housing, employment, disposable income, social networks etc and acts to accommodate these where they impact on a client’s health and well-being and ability to adhere to treatment or negotiate safe sex practices.
QPP's Case Managers provide regular support to clients with complex needs, such as those with mental health or substance misuse issues and harder to engage clients such as those with unstable accommodation, those from Aboriginal and Torres Strait Islander communities, and people from culturally and linguistically diverse (CALD) backgrounds.
