Maroochydore Community Health

The Continuing Care team based at Maroochydore offer a confidential service for individuals over the age of 18 living on the Sunshine Coast who experience moderate to severe impairment in functioning due to mental illness and have difficulty accessing and maintaining psychosocial supports. The Continuing Care team is guided by a multidisciplinary case management model of care.
Current clients of the Maroochydore Continuing Care team can contact the service between 8:00am - 4:30pm Monday to Friday on (07) 5459 6900 to speak to their Case Manager.
New referrals to the Sunshine Coast Mental Health Service are encouraged to contact the Mental Health Access Service on 1300 MH CALL (1300 64 22 55). This service is available 24 hours a day.
