NDIS Support Coordination

If you are aged under 65 years and have a permanent and significant disability, you may be eligible for the National Disability Insurance Scheme (NDIS) services. The NDIS provides support for people with a disability to achieve their goals, participate in their community, and be part of the workforce.  We provide an extensive range of supports, either through direct services or by partnering with other providers. By doing this, we can be a single point of contact for you.

Our diverse range of NDIS services enables us to adjust your care plan as your needs change. We provide support, nursing, and allied health services (such as physiotherapy), and have several therapists that are part of the Co.As.It. team, making us more responsive and effective than other providers. Whatever your situation, we can offer you the support you need. We will be supportive, responsive, adaptive, and innovative in your choice of care and lifestyle. You will have direct access to your dedicated coordinator who will manage your care choices.
